Transitions Home Care has been created with a clear focus on the needs of our clients Through a structured client discovery process, we have validated the key problem as To maintain a collaborative linkage with other programs serving the DD population.. This is a critical issue for our selected target market of High risk DD adults.

Transitions Home Care is uniquely positioned to deliver value to High risk DD adults. "Transitions Home Care will Provide effective and efficient services using proven an innovative methods, as a result our client will see the value in there quality of life.." In order to deliver on this value proposition, Our training methods not only teach our staff on how to deal with people with DD . It all so boost morale so that there isn't a high turnover of staff. They are taught how to build relationships with not only the client but the family s well.. This capability has taken considerable time and resources to develop and forms the cornerstone of our competitive advantage.
Competitors
We have a number of competitors within the Developmental Disabilities Services market. We consider our key competitors to be: The larger Agencies i.e. Via Quest, Total Home Solutions, Blackstone. Our understanding of these competitors has been used to refine our strategy and value proposition. We believe that we can compete effectively with the larger agencies by keeping our numbers small .Because it's not just about making money , It's providing quality service . It' will be helping the the DD community become successful being integrated in what we call normal society..
Key Strategies
Over the next 6 months, our focus will be to Appoint a board of governors.. This task will be undertaken in the context of our broader strategy to Expand to serve all 88 counties of the state of Ohio by touching each county .. These strategies are a tactical implementation of our overall business goal as outlined above.
Milestones
Measurement of the successful implementation of our strategies will be governed by measurable development milestones. The milestones that will be achieved over the next 2 years are: Obtain 8-10 / 24 hrs clients in each county. With the possibility of expanding to Kentucky and Indiana.
